With the support of the community, the Dementia Care and Education Campus, an innovation of Hospice of the Valley, is a comprehensive resource for those living with all stages of Alzheimer's disease and other dementias. The Campus will provide full-spectrum care for persons living with mild, moderate, or advanced dementia and support for their families. A 12-person Assisted Living Center, a 24-member Adult Day Club with adjoining Child Center, and a 12-person Hospice Inpatient Care Home will provide superb and creative person-centered care. This person will provide person-centered personal care (bathing, continence care, nutrition, ambulation) according to the care plan developed by the interdisciplinary team. **Responsibilities** + Provides engaging person-centered activities for adult day club members + Performs quality individualized personal care. + Contributes to the warm and welcoming atmosphere of the unit. + Assures patient, staff and visitor safety. + Creates timely and accurate documentation. + Maintains and enhances professional skills. **Minimum Qualifications** + Current Arizona C.N.A.or Caregiver. + Current CPR certification. + Current Arizona Fingerprint Clearance card. + Food handler's Card. + Excellent communication and interpersonal skills. + Solution driven, creative and resourceful problem-solving skills. + Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through. + Proficient computer skills. **Preferred Qualifications** + Previous dementia care experience.
