Job Description Summary:

The Medical Social Worker will provide psychosocial support, counseling, and resource coordination to patients and their families to address social, emotional, and practical concerns related to illness, disability, and end-of-life care. The ideal candidate will have a strong understanding of medical and psychosocial issues, excellent communication skills, and a commitment to holistic, patient-centered care while reporting to the Director Of Nursing (Hospice).

Job Duties (include, but are not limited to):

  • Conduct comprehensive psychosocial assessments of patients and families to identify strengths, needs, and preferences related to medical, social, emotional, and financial aspects of care.
  • Provide counseling, emotional support, and crisis intervention to patients and families coping with illness, disability, loss, and end-of-life issues.
  • Develop individualized care plans in collaboration with interdisciplinary team members to address psychosocial concerns, treatment goals, and support needs.
  • Assist patients and families in accessing community resources, financial assistance programs, and support services to enhance their well-being and quality of life.
  • Facilitate communication and decision-making among patients, families, caregivers, and healthcare providers to promote effective care coordination and continuity of care.
  • Advocate for patients’ rights, preferences, and autonomy in accordance with ethical and legal standards.
  • Document assessments, interventions, and outcomes accurately and in accordance with regulatory requirements and professional standards.
  • Participate in interdisciplinary team meetings, case conferences, and care planning discussions to contribute insights and recommendations from a social work perspective.

Position Qualifications:

  • Master’s degree in social work from an accredited program.
  • Current licensure or certification as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) in the state of practice.
  • Minimum of 2-3 years of experience in medical social work, preferably in home health or hospice care.
  • Strong knowledge of medical terminology, psychosocial assessment techniques, and community resources.
  • Excellent communication and interpersonal skills, with the ability to establish rapport, build trust, and facilitate difficult conversations.
  • Compassionate and empathetic approach to patient care, with a commitment to cultural competence and diversity.
  • Ability to work independently as well as collaboratively in a multidisciplinary team environment.
  • Proficiency in computer applications and electronic health record (EHR) systems.
  • Prudent use of office supplies and equipment.
  • Ability to communicate effectively.
  • Must have reliable transportation.
  • May require occasional weekend, holiday, or overtime work.
  • Skills in problem-solving, considering patients' backgrounds, finances, and resources, are essential for delivering optimal care. 
  • Compliance with dress code and professional demeanor.
  • Ability to communicate effectively in English.
