Job Description

Step into the role of a Medical Social Worker and become a vital link between patients, families, and the care team, guiding them through complex transitions with empathy, evidence-based planning, and resourceful advocacy. In Southgate, Michigan, you will shape care journeys that honor dignity, cultural needs, and social determinants of health while building resilience across diverse populations. This is a chance to translate clinical insight into tangible support—assessing needs, coordinating services, and connecting patients with essential resources to improve outcomes. The opportunity is designed to be professionally enriching, offering you a pathway to deepen your clinical impact while maintaining the human-centered focus that defines exceptional social work. And for those drawn to broader experiences, there are possibilities to work in carefully selected locations across the United States, expanding your professional horizon while you continue to refine your specialty in medical settings.

Southgate sits in the Detroit metropolitan region, a mosaic of neighborhoods that blend urban culture with suburban charm. You’ll enjoy convenient access to parks, riverfront trails, and lakeside escapes along the Great Lakes, alongside a thriving arts and dining scene. Michigan’s natural beauty provides quiet spaces for reflection after demanding days—from strolls along the Detroit Riverwalk to family outings at nearby parks. As you gain familiarity with this community, you’ll also discover opportunities to travel to other communities within the network, experiencing a range of hospital, clinic, and post-acute environments that broaden your clinical perspectives and deepen your competencies as a medical social worker.

In this position, you’ll play a central role in patient-centered care. Your responsibilities include comprehensive psychosocial assessments, discharge planning, and risk assessment to ensure safe transitions from hospital to home or alternative care settings. You’ll coordinate with interdisciplinary teams to align treatment plans with patient goals, identify and address barriers to care, and connect patients and families with housing, transportation, financial assistance, caregiver support, and community resources. Your role extends to crisis intervention, ethical decision-making, and culturally sensitive communication, with meticulous documentation to support care continuity. You’ll guide families through complex decisions, advocate for patient rights, and help patients navigate insurance, palliative and hospice options, and caregiver needs. This is a role that rewards judgment, empathy, and creative problem solving, with pathways for professional growth through mentorship, cross-training, and ongoing education in areas like case management, trauma-informed care, and palliative support. The environment promotes collaboration with physicians, nurses, therapists, and social work peers to deliver holistic care that respects patient values and strengthens families’ resilience.

The compensation package recognizes the expertise you bring. This short-term contract offers a competitive weekly pay range of $566-$611, with a start date of 02/16/2026 and assignments planned in weeks, with extension opportunities based on performance and program needs. While many assignments provide rewarding of support and growth, there is no guaranteed minimum number of hours, so the schedule can adapt to patient flows and facility requirements. In addition to competitive pay, you can access a comprehensive benefits package that may include a bonus, housing assistance or stipends, and options for contract extensions that let you deepen ties with the organization and gain exposure to additional clinical settings. And because you’ll be traveling as part of this program, you’ll receive 24/7 support from a dedicated team to address travel logistics, work-site questions, and clinical coaching whenever you need it.

Our company is built on empowering staff to grow, learn, and lead with confidence. We invest in your career advancement, providing structured pathways for promotion, access to continuing education credits, and opportunities to broaden your scope across multiple medical environments. A supportive, collaborative culture underpins every assignment, with teammates who value your voice, celebrate your clinical judgment, and partner with you to design feasible plans that improve patient outcomes. You’ll join a community that prioritizes work-life balance, ethical practice, and a commitment to equity, where your professional development is as important as patient care.

If you’re driven to shape meaningful outcomes for patients during vulnerable transitions and to build expertise within medical social work, this is your invitation to apply. Embrace the chance to serve in Southgate, explore Michigan’s scenic landscapes, and potentially experience diverse sites across the country, all while benefiting from strong support, purposeful work, and a clear path for growth. Ready to amplify your impact, expand your practice, and contribute to a team that values your dedication? Apply now to join us in advancing compassionate, patient-centered care.

Note: The hours and pay rates listed are estimates and may vary. Final compensation packages and guaranteed hours will be confirmed during the hiring process.
